Accuracy of Ultrasound Guided Versus Blind Knee Intra-articular Injection for Knee Osteoarthritis Prolotherapy

چکیده

Knee osteoarthritis (OA) is one of the most common joint disabling diseases among adult patients. Symptomatic knee OA occurs in 10% men and 13% in women aged 60 years or older but is likely to increase due to the aging of the population and the obesity epidemic [1]. Osteoarthritis (OA) is characterized by progressive loss of articular cartilage, subchondral bone sclerosis, osteophyte formation, changes in the synovial membrane, and an increased volume of synovial fluid with reduced viscosity [2]. It is a complex disease with unknown etiology but evidence is growing for the role of systemic factors (such as genetics, dietary intake, estrogen use, and bone density) and of local biomechanical factors (such as muscle weakness, obesity, and joint laxity) [3]. Current mainstay of microinvasive therapy is prolotherapy by intra-articular injections. Prolotherapy with dextrose and with prolozone resulted in the same pain relief or functional improvement in patients with mild to moderate knee OA [4]. A randomized controlled trial (RCT) reported significant improvement in knee OA pain scores when treated with prolotherapy [5]. Prolotherapy results in progressive improvement of knee pain, function and stiffness in long-term follow-ups [6]. Blind intra-articular injection is still the choice of injections for many clinicians with acceptable success rate [7]. However, it could result in extra-articular approach in some patients. Besides, accidental collision of the needle tip with the superolateral pole of the patella during attempted aspiration or injection of the knee joint could led to pain, as well as damage to the chondral cartilage of the patella [8]. Although, ultrasound has been recommended for intra-articular injection, however its accuracy only been tested on cadaver.

Comparative effectiveness of intra-articular prolotherapy versus peri-articular prolotherapy on pain reduction and improving function in patients with knee osteoarthritis: A randomized clinical trial

Background Osteoarthritis is a common degenerative disease. Prolotherapy is an alternative therapy used in multiple musculoskeletal disorders. Objective To compare the effectiveness of intra-articular dextrose injection versus peri-articular prolotherapy in patients with knee osteoarthritis (KOA). Methods Fifty-two adults with painful primary knee osteoarthritis for at least three months we...

A comparison of ultrasound guided corticosteroid injection vs ozone injection in grade 3 knee osteoarthritis

Background: Osteoarthritis (OA) is a degenerative disease that can cause pain and limited function. One of the non-surgical treatments is intra-articular ozone injection. This study aimed to compare the effect of ultrasound-guided corticosteroid injection with ozone injection in patients with grade 3 osteoarthritis of the knee. Methods: This randomized, double-blind, clinical trial was carried...

The effects of injecting intra-articular platelet-rich plasma or prolotherapy on pain score and function in knee osteoarthritis

Introduction Osteoarthritis (OA) is a chronic joint disease that usually occurs in older people and leads to pain and disabilities. OA treatment ranges from drug therapy to surgery. Drug and rehabilitation therapy are preferred over surgery, and, especially, there is a tendency toward compounds causing regenerative changes in the knee joint. In the present study, the effects of platelet-rich pl...

Periarticular dextrose prolotherapy instead of intra-articular injection for pain and functional improvement in knee osteoarthritis

BACKGROUND Osteoarthritis (OA) is a degenerative disease that can lead to painful and dysfunctional joints. Prolotherapy involves using injections to produce functional restoration of the soft tissues of the joint. Intra-articular injections are controversial because of the introduction of needles into the articular capsule. OBJECTIVES To compare the effect of periarticular versus intra-artic...

Clinical utility of ultrasound guidance for intra-articular knee injections: a review

Intra-articular corticosteroid and hyaluronic acid injections provide short-term symptom amelioration for arthritic conditions involving structural damage or degenerative changes in the knee. Conventional palpation-guided anatomical injections frequently result in inaccurate needle placement into extra-articular tissue and adjacent structures. The purpose of this review was to determine the eff...
